Affinity Adult Program
Affinity is a Section 21 and 29 Community Based Day Habilitation Provider. Affinity will provide a combination of one-on-one and small group support to facilitate individuals with developmental disabilities desire for independence and community participation. After case management makes the referral and the service is approved by DHHS, Affinity will meet with the focus person and their community support team. At this meeting, an individual service plan will be designed to address the areas of need and desires of the focus person. The focus person will then be carefully matched with a Direct Support Professional to work on the individual goals outlined in the service plan. The Direct Support Professional will pick up the focus person at their home and then head out into the community to work their chosen activity.
The focus person will be encouraged to actively engage in daily decision making, healthy living choices, and mutually beneficial and appropriate community relationships. Affinity will work with the focus person on decreasing his/her need for professional support through increasing his/her independence, ability to self- advocate and through building new skills and natural supports. Affinity will work closely with the focus person and other members of his/her community support team ensuring that their aspirations as expressed in his/her Service Plan are being met.
